Resumen
El trabajo revisa la situación actual del sistema de salud de Venezuela. Su propósito central es proponer una agenda de temas críticos que deberán ser considerados en detalle para establecer pautas de reforma. Para la revisión se utiliza como referencia inicial la perspectiva y metodología propuesta en 1940 por José María Bengoa sobre la medicina social, con especial énfasis en la realidad del medio rural venezolano. Esta referencia es complementada con nuevos aportes en el análisis de políticas de salud, como es el concepto de sistema de políticas. La utilización de esta última perspectiva permite ilustrar que en Venezuela existen importantes limitaciones en los sistemas de políticas generales y específicos, con lo cual se afectan las condiciones de salud y se dificulta la implementación de reformas adecuadas. En líneas generales, las políticas de salud en Venezuela no garantizan la atención ni de lo urgente ni de lo importante. El trabajo concluye que es fundamental acometer cambios sustantivos que mejoren el actual desempeño del sistema de salud, especialmente en las áreas de financiamiento, organización y prestación de servicios.

Health policy in Venezuela: Neither the urgent nor the important

Abstract
The paper reviews the current situation in the Venezuelan health system. The main objective of the paper is to propose a general agenda of critical issues to be considered in the definition of policy options. The review takes into consideration the perspective and methodology formulated by José María Bengoa in 1940 regarding the role of Social Medicine, in the specific context of rural Venezuela. This approach is combined with recent developments in the analysis of health policies, such as the concept of policy systems. The application of this concept illustrates that the performance of the Venezuelan health systems is strongly affected by serious limitations in the general and specific policy systems. As a result, health conditions at the country level show signs of deterioration and stagnation. In general terms, health policies in Venezuela do not respond adequately neither to the urgent nor important matters. The paper proposes fundamental changes aimed to improve the current performance of the Venezuelan health system, especially in the areas of financing, organization and provision of services.
